1. If phoning in or faxing your order, print out order form on your printer.
2. Fill out your complete mailing information.
3. Specify Basket pattern, kit, featured basket, or accessory name, along with
    all other details.  Color chart below.
4. Add price of all items, shipping & handling, and tax charges (Ohio residents only)
    for Total Amount Due.
5. If mailing, fill in credit card information or make check or money order out to
    Cornerstone Baskets.  Send to:
